How to apply for Personal Plus

To apply for Personal Plus coverage, follow these instructions:

  1. Complete and sign the Personal Plus Enrollment Form (PDF). To complete the form, you must select a primary care physician for each family member. Locate a physician in Find a doctor .
  2. If you are employed and your employer does not offer health care coverage, get a letter from your employer stating this fact.
  3. Mail the completed enrollment application and required documents to:

    Detroit, MI 48226
  4. You also may “fax” the completed form to 313-983-2286. Only completed applications are accepted at this number.

Enrollment applications are accepted year-round, and approval is contingent upon your passing our underwriting guidelines. If you are accepted, we will mail an invoice to you for your premium.
