How to apply for Personal Plus

To apply for Personal Plus coverage, follow these instructions:

  1. Complete and sign the Personal Plus Enrollment Form (280K PDF). To complete the form, you must select a primary care physician for each family member. Locate a physician in Find a Physician.
  2. Fill out a health questionnaire (1,057K PDF) for each family member.
  3. If you are employed and your employer does not offer health care coverage, get a letter from your employer stating this fact.
  4. Determine your monthly premium. Issue a check or money order payable to Blue Care Network of Michigan for your first two months' premium.
  5. Mail the completed enrollment application, health questionnaires, the letter from your employer (if applicable) and your check or money order to:

Enrollment applications are accepted year-round, and approval is contingent upon your passing our underwriting guidelines. If you are not accepted, your premium will be returned.
