How to apply for Personal Plus
To apply for Personal Plus coverage, follow these instructions:
- Complete and sign the Personal Plus Enrollment Form (PDF). To complete the form, you must select a primary care physician for each family member. Locate a physician in Find a Physician.
- Fill out a health questionnaire (PDF) for each family member.
- If you are employed and your employer does not offer health care coverage, get a letter from your employer stating this fact.
- Mail the completed enrollment application, health questionnaires and required documents to:

Enrollment applications are accepted year-round, and approval is contingent upon your passing our underwriting guidelines. If you are accepted, we will mail an invoice to you for your premium.
