How to apply for OneBlue℠
- Complete and sign the enrollment form (PDF). Select a primary care physician for each family member from the BCN online provider directory.
- Determine your monthly premium by using the rate table. Send a check or money order payable to Blue Care Network for the first month's premium.
- Mail the completed enrollment form and check to:
