  1. Complete and sign the enrollment form (PDF). Select a primary care physician for each family member from the BCN online provider directory.
  2. Return completed enrollment form to:

    Enrollment applications are accepted year-round, and approval is contingent upon your passing our underwriting guidelines. If you are accepted, we will mail an invoice to you for your premium.
