Rights and responsibilities of members in living with illness programs
Our living with illness programs are built on a partnership between members, physicians and Blue Care Network. As a member enrolled in one of our living with illness programs, you have rights and responsibilities.
You have the right to:
- Choose not to participate by calling Disease Management at 800-392-4272, from 8:30 a.m. to 5 p.m. Monday through Friday (except holidays).
- Obtain the name of the manager who is responsible for overseeing the living with illness program in which you participate so that you can request more information, ask for clarification or request a change in either the program or your participation in the program.
- Be informed of all living with illness related treatment options included in BCN's clinical guidelines and be able to discuss them with your physicians, regardless of the cost or your benefit coverage.
- Receive information that is understandable.
You have the responsibility to:
- Notify BCN and your physician if you decide not to participate.
- Follow the directions of your physician and the general care and educational guidelines offered by BCN as part of your living with illness program efforts.
- Provide, to the fullest extent possible, complete, accurate and timely information to your physician(s) in order for proper care to be delivered and to BCN in order to support the care delivery process.
