Before surgery

In some cases, if you are scheduled for surgery you will receive a phone call from a nurse before surgery. During this call, the nurse will discuss:

If you are hospitalized

If you are admitted to the hospital, a nurse will follow your hospital stay and assist in coordinating your care after you are discharged.

Our case management programs

For members who require more complex care, we offer the following programs:

High-Risk Pregnancy:

This program helps those experiencing complications with a pregnancy to a full-term delivery while minimizing the risks to you and your baby.

Neonatal Care:

Our case managers follow premature infants admitted to a neonatal intensive care unit after delivery and work with parents for up to one year following the baby's discharge from the hospital.

Organ Transplant:

Transplant case managers work closely with your primary care physician, your specialists and you to identify appropriate transplant centers of excellence and coordinate the services you need before and after the transplant.

Oncology:

Case managers assist with your treatment plan and offer procedures that may help minimize side effects.

Frequent Emergency Room Use:

Our case managers assist in coordinating your care with your physician.

Serious Illness:

Our case managers work with you and your physicians to coordinate all care and services necessary.
