Healthy Blue Living Rewards℠ can help you save money
Healthy Blue Living Rewards is Michigan’s first outcome-based HMO plan that rewards you with lower health care costs for achieving healthy results. Healthy Blue Living Rewards is an extension of Blue Care Network’s successful Healthy Blue Living product.
You can lower your copayments and deductible by meeting the health targets. The plan offers three benefit levels:
- Enhanced – Enhanced benefits are awarded to those who achieve the plan’s health measures. You save money when you use your health plan services by receiving a lower or no deductible and lower copayments.
- Intermediate – Intermediate benefits have an average level of copays and deductible and are awarded to those who may have a health condition but are committed to working toward becoming healthier.
- Standard – Standard benefits are given to those who chose not to meet the plan requirements or participate in physician prescribed treatment to improve their health. You still have health care coverage but with higher copays and a higher deductible.
The health plan concentrates on six high-impact health measures:
- Tobacco
- Weight
- Blood pressure
- Cholesterol
- Depression
- Blood sugar
Why these? They have a huge impact on illness and fitness. Managing them will decrease your chances of developing one or more chronic or disabling diseases.
Healthy Blue Living Rewards focuses on wellness and prevention. It gives individuals the knowledge, skills and support to achieve their health goals.
