Member to-do list
Use this handy list to check off the initial steps you need to take to qualify for enhanced benefits. You’ll be rewarded with lower copays and deductibles when you do.
Initial requirements
- Make sure you’ve selected a Blue Care Network primary care physician. If you haven’t, visit MiBCN.com/find or call Customer Service.
- Qualification Form: Visit your Blue Care Network primary care physician to complete a Qualification Form, scoring at least 80 points. Deadline: 90 days from the start of the plan year.
- BCN health assessment: Complete an online health assessment at. Deadline: 90 days from the start of the plan year.
You may also have these requirements:
- Quit the Nic: If you smoke, enroll in our Quit the Nic smoking cessation program (800-811-1764). Deadline: 120 days from the start of the plan year.
- Weight management: If your BMI is 30 or above, complete BCN’s two online weight management questionnaires. Deadline: 120 days from the start of the plan year.
