What you need to do to continue receiving enhanced benefits
You automatically receive enhanced benefits with lower copayments and deductibles for the first 90 days of coverage. To continue receiving enhanced benefits, within 90 days of enrollment, you and your spouse need to adopt a healthy lifestyle and complete:
- A Qualification Form (PDF) – Complete this form with assistance from your primary care physician and send it to BCN. Mailing address and fax number are listed on the form.
- An online health assessment – Learn about your health risks and plan for better health.
