Bariatric Centers of Excellence
In order for the service to be covered, bariatric surgery must be preauthorized by Blue Care Network and performed at a Network 1 Bariatric Center of Excellence, or the University of Michigan. Covered hospitals include:

- Hurley Medical Center, Flint
- St. John Macomb-Oakland Hospital, Madison Heights
- University of Michigan Health System, Ann Arbor
- William Beaumont Hospital, Royal Oak
Cardiac Centers of Excellence
- Genesys Regional Medical Center, Grand Blanc
- McLaren Regional Medical Center, Flint
- Mount Clemens Regional Medical Center, Mount Clemens
- Providence Hospital & Medical Center, Southfield
- St. John Hospital & Medical Center, Detroit
- St. John Macomb-Oakland Hospital, Warren
- St. Joseph Mercy Oakland Hospital, Pontiac
