To make sure you receive all the benefits you are entitled to, you must keep your records accurate. Make sure you report changes to us within 30 days of their occurrence. It’s particularly important that you promptly report events, such as the birth of a child, as soon as possible so we can process the change and pay your claims. Once the paperwork is completed, your contract will register the change as of the date of the event. It’s also important to keep your address up to date.
If you have group coverage, contact your group benefits representative to make changes to your records. If you are covered under a nongroup policy, call Customer Service at 1-800-662-6667. The representative can tell you what documentation to send. You are required to report the following events within 30 days of the event:
