How to Use Your Benefits

Present your Blue Care Network identification card each time you need health care services. The information on your ID card tells your health care provider what kind of coverage you have. This is especially important if you are covered by more than one health plan and therefore have more than one ID card.

We supply ID cards for each of our subscribers and additional cards for covered spouses, domestic partners and dependents over the age of 18. We do not issue separate ID cards for children under the age of 18 — parents must present their ID cards on behalf of their children.

If you are seeking care from a noncontracted doctor, call the authorization number on the back of your identification card before you receive care to have your treatment covered. Some care requires preauthorization for it to be a covered benefit. See the Your Benefits section of your Blue Elect SRO Certificate.

When you use a noncontracted doctor, he or she may require payment before you receive services. When you pay for services you will need to submit for reimbursement using the Member Reimbursement form (95K PDF). Reimbursement will be based on BCN's fee schedule and may be less than what the doctor charged.
