Continuity of care
If your primary care physician or specialist leaves our network, you may continue your care with him or her under certain conditions.
In the case of specialist care, you must be receiving care for an ongoing course of treatment and disruption of that care would interfere with recovery. For PCPs and specialists, you may continue:
- Care for up to 90 days after your doctor notifies you the participation agreement has ended
- Through postpartum care, if you are in the second or third trimester of pregnancy when your doctor’s relationship with Blue Care Network ends
- For the rest of your life, if you are being treated for terminal illness prior to the doctor’s leaving BCN
Continuity of care is not automatic. Your doctor must choose to continue to care for you and must notify you of that intent in writing. Your doctor must also agree to accept our reimbursement as payment in full and follow our quality standards, utilization review policies and procedures for referrals and pre-authorizations during the transition period.
