The Evidence of Coverage gives details about your Medicare health coverage and explains how to get the care you need. The EOC is an important legal document and is our contract with you. It explains your rights, benefits and responsibilities as a member of BCN Advantage. It also explains our responsibilities to you.

Evidence of coverage for individual members

Individual members can also refer to a Summary of Benefits (PDF) that describes the available benefits and monthly premiums for BCN Advantage Option 1, Option 2 and Option 3. The Summary of Benefits does not list every service we cover, every limitation or every exclusion.




BCN Advantage is a Medicare-approved HMO. BCN Advantage has a contract with the Federal Government that is renewed annually.
