Based on your income and resources, you may qualify the Low Income Subsidy — Extra Help with paying for your Part D prescription drug premiums and costs.
If you are an individual (non-group) plan member, you can contact Social Service Coordinators for assistance in applying for the Low Income Subsidy (Part D Extra Help). Social Service Coordinators contracts with BCN Advantage to provide free education and assistance to enroll individual (non-group) plan members into an array of federal, state and local social programs.
