Did you know that a pneumococcal vaccination can protect you against the germ that causes pneumonia? This vaccination is recommended for all individuals age 65 and older. While the vaccine will last most people a lifetime, you may need to receive a periodic booster shot.

To receive a fleece lap blanket:

1. Download the incentive form and print it out.
2. Fill out Section 1.
3. Get a pneumococcal vaccination in 2009. Ask your doctor or a test site provider to fill out and sign Section 2. (If you’ve already had a shot this year, ask your primary care physician to fill out and sign Section 2 of the form.)
4. Mail or fax the form to us.

To receive your reward, you need to get your vaccination by December 15, 2009. Your completed form also must be completed, signed and postmarked or faxed by that date.
