Most people have two kidneys located near the middle of their back, just below the rib cage. The kidneys filter waste products from the blood. They maintain proper levels of minerals such as salt, phosphorus, calcium and potassium. They also remove excess water in the form of urine.

What kidney disease is

When your kidneys don’t remove waste, these waste products build up in the blood and damage your body. The damage usually occurs very gradually, and symptoms aren’t obvious in the earlier stages. Kidney disease can be caused by diabetes and high blood pressure. Some kidney diseases run in families.

You can manage your kidney condition

Kidney disease is a chronic medical condition that will not get better on its own. You can manage your condition and live an active life by following a treatment plan that includes the following:

Chronic Kidney Disease Management Program

The Chronic Kidney Disease Management Program is available to all our members 18 and older who have been diagnosed with kidney disease. If you have been diagnosed with kidney disease, you will be automatically enrolled.

This program will help you understand kidney disease, how it affects your life and how you can better manage your condition. A few times each year, you’ll receive our newsletter, which will help you:

Case managers provide individual education and counseling, and coordinate care with physicians, for members who need complex care.
