Rewards for BCN Advantage members

Our Healthy Advantage Rewards program is our way of thanking you for taking care of your health.

It's easy to participate. Just take action on any of the four Healthy Advantage Rewards initiatives, fill out the corresponding form and send it back to us. These initiatives are so important, we'll send you a different Entertainment^® coupon booklet for each form you complete, as appropriate for you.

Physician assessment – The physician assessment is a form your doctor can complete after an exam with you. Once we receive the form from your doctor, we'll send you a 96-page Travel, Dining and Movies coupon booklet. This is the only form your doctor must submit on your behalf.

Advance directive – Give a copy of your advance directive to a responsible party, return the rewards form to us and receive coupons for local merchants in your ZIP code.

Flu vaccine – After you receive your seasonal flu shot in 2011, return the form to us and we'll send you a 96-page Michigan Savings coupon booklet.

• 2011 Flu Vaccine Reward form

Pneumococcal vaccine – This shot can help protect you from pneumonia. While the vaccine will last most people a lifetime, you may need a periodic booster shot. If you need a shot or booster and get it this year, we'll send you a 128-page National Values coupon booklet.
