Monitoring your diabetes helps you stay in control. Each year, people with diabetes should have several tests as part of their routine care. These four tests are so important we’ll reward you for getting them by December 15, 2009.

To receive a fleece lap blanket:

1. Download the incentive form and print it out.
2. Fill out Section 1.
3. Schedule the tests you need with your primary care physician or eye care specialist. Ask your doctor or eye care specialist to fill out and sign Section 2. (If you have already had some or all of these tests this year, ask your doctor to complete and sign Section 2. Schedule any remaining tests.)
4. Mail or fax the form to us.

To qualify, you need to get all four tests by December 15, 2009. Your completed form also must be signed and postmarked or faxed by that date.
