Heart failure management program

Health checks

Every year, ask your doctor about these wellness checks. You may need some of them more than once a year.

  • Blood pressure
  • Total cholesterol (HDL, LDL, triglycerides)
  • Monitor your weight
  • Flu shot

Also, check with your doctor to see of your pneumonia shot is up to date.

Heart failure happens when the heart muscle becomes weak and does not pump your blood as well as it should to supply your body with the oxygen and nutrients it needs. Our heart failure program is part of our Disease Management program. We want to help you understand heart failure and learn how to live well with it.

Managing heart failure

Despite the name, heart failure doesn’t mean your heart is no longer working or could stop working soon. By following your treatment plan, there’s a lot you can do to live well. You can manage your condition and work with your primary care physician to:
