The Heart Failure Management Program is available to all our members 18 and older who have been diagnosed with HF that is not correctable with surgery. If you have been diagnosed with noncorrectable HF, you will be automatically enrolled.
This program will help you understand heart failure, how it affects your life and how you can better manage your condition. A few times each year, you’ll receive our Lifestyles for Heart Disease newsletter, which will help you:

Case managers provide individual education and counseling, and coordinate care with physicians, for members who need complex care.
