  1. Log in to Member Secured Services. If you have not already done so, you will need to register. Your health plan ID card has information you’ll need. If you have any questions about login or registration call 888-417-3479.

    IMPORTANT: Each member taking the health assessment must log in with their own ID and password for us to have a record of the completed assessment.
  2. Click the health assessment link.
  3. Complete the health assessment and you'll receive your lifestyle score and a personalized plan that includes what you're doing well and what you can do better.
