How to take a health assessment
View your benefits, check the status of a claim, order ID cards and more!
- Log in to Member Secured Services. If you have not already done so, you will need to register. Your health plan ID card has information you’ll need.

IMPORTANT: Each member taking the health assessment must log in with their own ID and password for us to have a record of the completed assessment. - Click the BlueHealthConnection® link.
- Click the Complete Health Assessment link.
- Complete the health assessment and click Submit. You'll receive your lifestyle score and a personalized plan that includes what you're doing well and what you can do better.
Information you will need
To get a more complete picture of your health be prepared with this information:
- Blood pressure
- Cholesterol level, HDL, LDL and triglyceride
- Blood glucose level
