Enrollment and eligibility guidelines

All documents that constitute the terms and agreements between Blue Care Network and your organization must be submitted to your BCN sales representative or independent agent at least 25 days before your effective date.

Enrollment requirements are determined by the number of eligible employees in your group as reported on your payroll record in the preceding and current calendar years. Eligible employees who elect Blue Cross Blue Shield of Michigan coverage will count towards your participation requirement.

The following participation calculation applies to groups with 50 or fewer eligible employees and will determine if your group meets our participation percentage requirement:
