Employer groups with fewer than 100 enrolled employees can choose one of our packaged health care plans. Please select:

- Group Plan Options 2–49 enrollees (PDF)
- Group Plan Options 50+ enrollees (PDF)
Groups with more than 100 enrolled employees can either choose one of our packaged health care plans or we will customize a plan to meet your needs.
How to obtain prices, make a purchase or discuss customization
- Employer groups with a potential enrollment of 100 or more employees — contact a Blues sales representative.
- Employer groups with a potential enrollment of fewer than 100 employees — contact a Blues independent insurance agent.
